Instructions for Filling Out

1. ※For the reporter, please fill in as required. For other fields, please provide complete information.

2. For the identification document number, please fill in your ID card number or passport number. For foreigners, please indicate your nationality and provide relevant identification documents.

3. If the representative is an appointed agent, please submit a letter of appointment. If the representative is a legal agent, please provide relevant proof of identification in copy. For applications involving personal privacy information, please provide proof of the relationship.

4. For corporations, organizations, offices, or businesses, please attach a copy of the registration certificate.

5. When accessing, copying, or reproducing the files, please abide by the regulations regarding file access, copying, and alteration, and refrain from damaging or altering the contents of the files.

6. The fee standards for accessing, copying, or reproducing the files are indicated in Attachment 1.
